There is no cost to become a participating facility of the Caps for Kids program network. The facility designates a person to supervise the program and agrees to abide by and adhere to Caps for Kids policies. The specific requirements are simple and straightforward:
1. Facility will give caps solely to eligible individuals (as defined by the Executive Board and Hospital Program Administration) at no expense to the recipient or his/her family. 2. Facility will not sell the caps to anyone. 3. Facility agrees to transfer caps to or exchange caps with Caps for Kids, Inc. 4. Facility agrees to cooperate with Caps for Kids in efforts to secure additional autographed headwear when feasible. 5. Facility must display Caps for Kids Membership Plaque within hospital. 6. Facility agrees that in the event it no longer wishes to participate in the program, or fails to comply with the Caps for Kids policies set forth in this letter and/or bylaws, the facility will return its inventory of hats, at its expense, to the executive director.
